It can happen, just odd that it wasn't picked up on the inspection really.
I have over the 40+ years or so that I've been mucking about with bikes and cars always used a paint pen to dot mark when re-fitting/tightening nuts and bolts especially on critical systems like suspension and braking systems. Different colours for subsequent work to ensure it's done.
I learned this from my dad who was a flight engineer in the RAF (starting on piston engines and early jet engines) before moving on to work at Britannia and Monarch airlines as flight engineer on Bristol Britannia then Boeing 720B jet. As he said "If i forget to tighten something up we crash at very high speed and in an all consuming fireball"
I never sell to friends or family as anything, even wear and tear is always seen as something you should have known about or your fault, just not worth it.
