People have been fitting transaxles upside down (relocating the drain plug) or opening them to flip their internals.
I wonder if making the engine turn the other way is a feasible alternative. I can't seem to think of it requiring any more (or less) than a custom (reversed) camshaft and a starting motor that turns the other way. I must surely be missing something here.

Oil pump, belt or chain tensioning being on the wrong side of the run, distributor bob weights, water pump, to name but a few issues. But it’s feasible if you were to overcome those kind of things I suppose.

You should habe a look on some of the fiat 850 tuning/engine swap threads. The 100 series 903cc can be found in factory trim spinning in both directions so swaps are common.
